Pregnancy
- Supported a client with a high-risk pregnancy through education, guidance, and ongoing support, helping her carry her baby to 36 weeks and safely deliver a healthy infant.
Postpartum Care
- A client experiencing severe pain following a cesarean section contacted her Nurse Home Visitor for guidance. After receiving education and support, she arranged transportation to the hospital and effectively advocated for herself by communicating her symptoms and medical history. She was diagnosed with appendicitis and an infection, requiring emergency surgery that same day. She is now recovering well at home, while her baby remained safely cared for by the grandmother and father during her hospitalization.
- A client who had not seen a primary care provider in more than two years, including throughout her pregnancy and her child's first year of life, established care with a new provider, completed recommended lab work, and began addressing her health needs.
Infant Growth and Development
- A family accepted a referral to Birth to Three Early Intervention services in May. Their child, who was experiencing developmental delays, began crawling one day after their first birthday in July.
- A child was referred to Birth to Three due to concerns about head and neck positioning and limited range of motion. During home visits, the Nurse Home Visitor demonstrated floor-time activities, positioning strategies, and massage techniques. The family consistently implemented these interventions while awaiting evaluation. By the time of the assessment, the child no longer met criteria for services, as no developmental or physical concerns were identified. This positive outcome was attributed to the family's commitment and the ongoing support provided through home visits.
Mental Health
- A client faced significant family stress when her husband experienced mental health challenges and social isolation. Drawing on coping and communication strategies reinforced during home visits, she helped him reconnect with support systems and pursue employment opportunities, contributing to improved family stability and well-being.
Economic Self-Sufficiency
- Two clients attended in-person job interviews in July and were hired on the spot.
- A client, her mother, and her one-year-old child secured permanent housing and moved into their own apartment. Prior to obtaining housing, they lived in overcrowded conditions with pest infestation and later stayed temporarily with relatives, sleeping on the floor. The family now has a safe, stable place to call home.
- A client who graduated from NFP three months ago has successfully secured and maintained employment as a Home Health Aide.
Graduation and Program Success
- NFP recently hosted two statewide graduation ceremonies in July to celebrate program participants who graduated between June 2024 and September 2026. Graduates attended with their children and family members, recognizing their commitment and achievements throughout the program.
